Position Title: Independent Living Caseworker ($20.50hr - $24hr)

ESSENTIAL FUNCTIONS:
- Successfully engage youth/clients in developing and implementing their child permanency and transition plan using positive youth development, trauma-informed, and culturally appropriate frameworks.
- A minimum of 20% of the working hours will be spent providing quality in-person goal planning, instruction, and case management services according to the youth's/client's identified level of service.
- Ensure up-to-date paper & electronic client records in compliance with program, county, state, and federal regulations and private grant requirements as applicable.
- Maintain a caseload of 12 youth and ensure 8 hours per week of face-to-face client IL services are provided. These hours can be obtained in the home, community, school, or VYH office. Hours will be clearly documented in Credible.
- Ensure the safety of each client assigned to them every 30 days by completing a minimum of one 30-minute face-to-face visit in the client's home and completing the formal safety assessment in CAPS. This should be done in accordance with the PA Dept. of Human Services (DHS) Code and DHS Bulletins.
- Complete all paperwork on a timely basis including intake, goal plans, case notes, quarterly reports, execute all required assessments.
- Coordinate services for clients with community providers, service providers, schools, and medical professionals.
- Clearly and concisely provide all necessary updates and recommendations to the court.
QUALIFICATIONS:
- They will possess a minimum of a BS/BA in social work with at least 1 year of case management experience (preferred) or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and at least two years of case management experience.
- An individual with the ability to identify with and adapt to the goals and philosophies of the independent living program. Possess sound judgment and knowledge of adolescent development and behavior.
- Possess knowledge of County, State, and Federal Laws in Child Welfare.
- Ability to transport youth to, as well as attend in-person IL Groups a minimum of 1x per week till 7PM and assist in transports to/from groups. Attendance will be required for virtual groups as needed.
- Display organizational skills and be highly adept at managing multiple tasks and assignments.
- Successful team members are creative thinkers, are flexible in their approaches to people and projects, and are capable of working with diverse groups of the population.
- Ability to attend a minimum of 5 cultural events per year with clients (including transportation to and from the event). Cultural events may be held at night, on weekends, or include overnight supervision of youth attending camps through Valley Youth House, the State, or other approved entity.)
- Complete the York County Office of Children, Youth & Families Caseworker Training program within 3 months of position obtainment.
- Sedentary work that primarily involves sitting/standing. X Occasionally
