Job Description
We are seeking an experienced Licensed Master Social Worker LMSW to join our team of healthcare professionals. The successful candidate will be responsible for providing psychosocial assessments, counseling, and case management services to our patients. Your primary duties will include:
- Completing thorough psychosocial assessments to identify the underlying causes of each patient's presenting problem and developing effective treatment plans.
- Counseling patients and their families on coping strategies and stress management techniques to improve their overall well-being.
- Collaborating with clinic staff and community agencies to coordinate care and ensure seamless transitions between treatment phases.
- Providing education and resources to patients and their families on advance directives, organ donation, and other relevant topics.
- Documenting all interventions and activities in each patient's clinical record.
Requirements
To be considered for this role, you must possess a Master's degree in Social Work from a program registered by the Department of Education, with specific training dealing with adult patients. You should also have a minimum of 1-3 years' post-master direct counseling experience with adult patients. Additionally, you must comply with and maintain all requirements for a valid, unrestricted MSW license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
