Responsibilities
As a Master's Level Social Worker with Valor Healthcare, Inc., your key responsibilities will include:
- Conducting thorough psychosocial assessments to identify the underlying causes of a patient's presenting problem.
- Developing and implementing individualized treatment plans that address the unique needs of each patient.
- Providing education and support to patients and their families on topics such as advance directives, community resources, and VA benefits.
- Collaborating with healthcare providers and other stakeholders to ensure seamless patient care.
- Documenting all interactions and interventions in a timely and accurate manner.
Requirements
To be considered for this role, you must possess the following qualifications:
- A Master's degree in Social Work from a program registered by the Department of Education.
- Current licensure as a Master's Level Social Worker in the state of desired employment or in any U.S. state or territory.
- Minimum of 1-3 years' post-master direct counseling experience with adult patients.
- Proficiency in written and spoken English.
- Strong computer skills, including EMR experience.
