Job Overview

Duties
- Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
- Provide direct patient care, including management of symptoms and pain relief.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate patient care.
- Implement infection control protocols to ensure the safety of patients and staff.
- Educate patients and families about disease processes, treatment options, and end-of-life care.
- Participate in clinical research initiatives aimed at improving hospice care practices.
- Support patients with developmental disabilities by adapting care approaches to meet their unique needs.
- Address urgent care situations promptly while maintaining a focus on patient comfort.
- Document all patient interactions accurately in electronic health records.
Requirements
- Current Nurse Practitioner license with certification in hospice or palliative care preferred.
- Strong knowledge of infection control practices and procedures.
- Experience working with individuals with developmental disabilities is a plus.
- Proficiency in conducting patient assessments and developing comprehensive care plans.
- Familiarity with urgent care protocols and infectious disease management.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
