As a V.A.L.O.R. (Veterans Assisting and Linking Others to Resources) Case Manager, you will play a vital role in supporting and advocating for veterans in need of assistance. Working within our organization, you will be responsible for providing comprehensive case management services, coordinating resources, and facilitating access to appropriate support systems for veterans facing various challenges.
Identify and connect veterans with relevant community resources, including healthcare services, mental health counseling, housing assistance, employment programs, educational opportunities, and legal aid.
Coordinates discharges for clients by collaborating with the client, family, health care team, and community resources.
Provides consultative services and technical assistance to staff regarding identification of needed supports and services for participants.
Identifies barriers to placement and initialization of programming to overcome the barriers.
Maintain accurate and up-to-date case records, document client progress, and prepare reports as required by organizational policies and funding sources.
Collaborate with a multidisciplinary team, healthcare professionals, legal experts, sheriff liaisons and community organizations, to ensure holistic care and support for veterans.
Conduct group education sessions and workshops on topics such as veteran benefits, financial literacy, and mental health awareness.
Stay informed about current trends, research, and best practices in the field of veteran support and actively participate in professional development opportunities.
Strong organizational abilities to manage multiple cases, prioritize tasks, and meet deadlines.
Empathy, compassion, and a non-judgmental approach to work with veterans facing challenging circumstances.
Ability to work collaboratively within a team and independently with minimal supervision.
Proficiency in computer skills, including case management software and Microsoft Office suite.
