*Job Summary*
As a Case Management Director, you will be responsible for coordinating patient care and providing support to individuals with complex medical needs. You will work closely with healthcare professionals to ensure the delivery of high-quality and efficient care.
*Duties*
- Conduct patient assessments to determine needs and develop individualized care plans
- Collaborate with healthcare providers to coordinate services and treatments
- Advocate for patients to ensure they receive appropriate care and resources
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and up-to-date medical records
- Provide guidance and support to patients and their families throughout the treatment process
*Qualifications*
- Minimum of two years of Case Management experience in utilization management, case management, discharge planning or other cost/quality management program.
- Graduate of a program of Registered Nursing.
- Current RN license in the state of Iowa or a multistate license allowing to work in the state of Iowa
Job Type: Full-time
Pay: $93,272.00 - $125,900.00 per year
