Hospital Social Worker – SEARHC (Southeast Alaska)

Position Summary:
Work as part of a multidisciplinary team to support hospitalized patients from birth to end-of-life. Provide discharge planning, resource coordination, financial/housing assistance, and supportive counseling. Collaborate with patients, families, and providers to ensure seamless transitions and holistic care.
Key Responsibilities:
- Assess biopsychosocial needs
- Coordinate discharge plans and community referrals
- Support patients with financial, legal, and long-term care needs
- Conduct swing bed assessments
- Assist with DME, advance directives, and crisis referrals
- Document patient interactions clearly and timely
- Participate in team meetings and ongoing training
Qualifications:
- BSW or MSW required
- 2 years hospital social work experience
- Valid Alaska driver’s license (or within 6 months)
- Strong communication, advocacy, and coordination skills
