Pay rate (Per Visit) $35-$85 Mileage
At Home with Van Dyk located in Ridgewood, New Jersey is seeking a Registered Nurse- RN. We are looking for a dynamic nurse has experience in a private pay home care agency. The individual will assist with all clinical services for At Home with Van Dyk in accordance with local, state, federal, the Commission on Accreditation for Home Care (CAHC), regulations that govern our home care agency, and be directed by the Director of Nursing to ensure that the agency is providing the highest degree of quality care and is always maintained. The nurse will demonstrate dynamic communication skills with professionals, families, and caregivers.
Responsibilities and Duties
- Performs required nursing visits in supervisory or direct care capacity.
- Documents the nursing and / or employee activities to reflect provided services to the patient.
- Supervises and evaluates the performance of field staff based on plan of care
- Orients, supervises and teaches nursing personnel for whom he / she has assigned responsibility as directed by DON.
- Assumes additional responsibility as assigned by the Director of Nursing.
- Conducts initial client assessment and reassessments and plans client's care.
- Demonstrates the ability to communicate effectively with other members of the healthcare team, client and his / her family members
- Communicates to the client and family implications of the client's health status
- Adheres to standard precautions, aseptic technique and infection control guidelines
- Maintains confidentiality of client assessment and records
- Utilizes time effectively, maintaining a consistent level of productivity
- Respects the rights, privacy and property of others always
- Identifies and utilizes appropriate resources
- Refers patients / patient families to appropriate community-based services.
- Present a well-groomed professional appearance always.
- Demonstrate an upbeat personality, patience and tact always.
- Must submit all documentation in a timely manner
- Instructor training sessions for CHHA training school
