We are looking for an experienced Advanced Practice Clinician (PA-C or NP) to work full-time at one of our community-based clinics. These clinics are part of VC's partnership with Humana.
The ideal candidate will have a passion for working with seniors and medically-complex patients. Clinicians lead a care team that provides multi-layered support to adults, primarily over the age of 65. The care team includes a Licensed Clinical Social Worker, Dietician, Medical Assistant and Registered Nurse support.

Position Details
- $10,000 starting bonus
- Plus $10,000 in relocation funds available to those who qualify
- New grads with prior nursing experience will be considered
- 100% outpatient
