Are you passionate about empowering individuals and families to improve their health, housing stability, and overall well-being? Join the Vancouver Housing Authority (VHA) as a Pathways Specialist and play a vital role in connecting our community members with essential resources and services!
We work closely with local governments and other community partners throughout Clark County to address issues of affordable housing and homelessness and help families break the cycle of poverty. As a Pathways Specialist, you’ll work with individuals at risk of adverse health or housing outcomes, conducting risk assessments, developing personalized plans, and helping clients navigate community resources. Your role is hands-on and impactful—you’ll provide education, remove barriers to care, and support people in achieving self-sufficiency.
Key Responsibilities
- Conduct intake and risk assessments to identify client needs.
- Provide one-on-one coaching to help clients improve their health and housing stability.
- Connect clients with vital services, including healthcare, financial assistance, and transportation.
- Assist clients in overcoming challenges such as paperwork, appointment scheduling, and compliance with housing programs.
- Follow up with clients through home visits, phone calls, and community outreach.
- Maintain detailed records and contribute to program evaluation efforts.
Pay and Benefits
- The starting salary for this position is $23.57 - $25.93 per hour. In compliance with the Equal Pay and Opportunities Act, the full salary range for this position is $23.57 - $33.00 per hour.
- Full-time, non-exempt, union-represented position, working in-person Monday - Friday.

Requirements
- High School diploma or equivalent.
- Minimum of two years’ experience in health education or chronic disease management.
- Experience working with low income, multi-ethnic populations preferred.
- Bilingual preferred.
- Knowledge of community resources in Clark County a plus.
- Must have valid driver’s license, access to a reliable vehicle, and have fewer than 3 moving violations in the last 3 years.
