Organization: VIP MS - Revenue Cycle Job Summary: JOB SUMMARY Responsibilities include ensuring that items (i.e., procedures, supplies, visits, and pharmaceuticals) that are charged through the E.H.R. system are described, coded and in compliance with coding guidelines and regulatory standards; ensuring the accuracy and completeness of charges captured for insurance and patient billing of inpatient and outpatient services. . KEY RESPONSIBILITIES • Performs analysis of medical documentation in the E.H.R. system to verify the accuracy of charges and overall content of the patient record • Ensures that the CPT and ICD codes chosen by provider are supported by the documentation • Verifies that documentation is completed and accurate • Queries provided regarding diagnosis and procedures, if documentation discrepancies are identified, to expedite chart completion • Performs analysis of charges on patient account as well as ensuring that all the appropriate charge adjustments are applied to the account • Verifies that vaccination administration units accurately represent the service provided • Verifies all performed procedures and E/M services are present on claim • Verifies and resolves discrepancies by utilizing the tools and resources available, medical record documentation, VIPMS Business Office and/or contacting the appropriate provider • Identifies deficiencies or discrepancies in nursing and/or physician documentation that affects billing and notifies the originator of the incorrect documentation to allow for correction and proactive improvements related to charge capture • Understands the relevance and impact of modifiers on reimbursement • Technology • Uses telephone and computers to facilitate the flow of communication throughout the office • Learns and applies new technical skills and systems as technology changes.
