Job Summary
We are seeking a Day Habilitation Counselor to join our team. As a key member of our staff, you will play a vital role in teaching adults with developmental disabilities independent living skills, personal awareness, and community access. You will work closely with our clients to develop and implement personalized habilitation plans, ensuring their needs are met and their goals are achieved.
Responsibilities
- Implement and monitor Day Habilitation Programs, working collaboratively with clients to achieve valued outcomes outlined in Individualized Service Plans (ISPs).
- Provide documentation on the implementation of each habilitation plan, ensuring accurate tracking and evaluation of progress.
- Evaluate feedback on habilitation plans in a timely manner, making adjustments as needed to ensure optimal results.
- Collaborate with Coordinators to address the unique needs of each individual, tailoring habilitation plans to meet their specific requirements.
- Provide full-time supervision and habilitative services in standalone therapeutic programs, promoting independence and self-advocacy.
- Deliver pre-employment training at local volunteer sites, focusing on travel and community safety skills.
- Perform other duties as assigned, contributing to the overall success of our organization.
Requirements
- Associate's degree preferred; minimum 2 years' experience in serving individuals with developmental disabilities required.
Working Conditions
- Able to work an awake first shift, with flexibility to adapt to changing priorities in a fast-paced environment.
- Must possess a valid NYS driver's license and be able to lift up to 25 pounds with or without reasonable accommodation.
- Willingness to work autonomously in the community, with intermediate Microsoft Word and Excel skills.
Compensation
The successful candidate will receive a competitive hourly rate of $18.00 per hour, reflecting the importance of this role in supporting our clients' independence and community involvement.
