What are the responsibilities and job description for the Day Program Clinician - Education position at Vanderheyden, Inc.?
Provide clinical treatment and case management services to those individuals enrolled in the Vanderheyden School Day program. Treatment includes, but is not limited to individual, family and group therapy. Day program Clinician is responsible for those tasks, which require skills, judgement, and decisions that involve clinical knowledge, training and experience. Day Program Clinician is responsible for planning, implementing, and guiding clinical treatment for the individuals assigned to them within Vanderheyden's Day Program and abiding by the individuals IEP.

Primary Duties & Essential Functions
- Participate in the Intake and Planning process which includes reviewing pre-placement.
- Provides individual, family and group therapy to all individuals assigned to them and provides crisis interventions as needed as per IEP.
- Provides assessment and treatment planning for all assigned individuals.
- Coordinates clinical and related support services with all internal and external service providers and makes referrals to community resources as needed.
- Schedules, facilitates and documents Treatment Team and service Plan Review meetings according to agency/department standards.
- Works in conjunction with appropriate Vanderheyden school staff to monitor and improve individual's academic performance, school behavior, attendance, and positive social emotional growth.
- Completes and maintains accurate documentation according to agency/department standards pertaining to progress/case notes, Psycho-socials, recommendation letters for court/referral source, IEP counseling goals.
- Participates in supervision.
- Maintains open communication with home district Committee on Special Education Chairperson regarding the process and performance of their students.
- Performs on-call consultation as assigned
- Participates in the PBIS Committee
- Assumes responsibilities that belong to all education staff members as participation in the behavior management program, ensuring the health, safety and supervision of students and enforcing the Student Handbook and Code of Conduct.
- Participates fully and effectively in agency committees, meetings, and/or teams as assigned.
- Acts as a liaison and/or representative for the agency and individuals served at case
- Assists and participates in activities, which improve the clinical, physical, social, emotional, and intellectual development of students.
- Contributes to and/or participates in clinically based projects and activities.
- Other duties as necessary.
- Master's degree from an accredited program in Social Work, Psychology, Rehabilitation counseling or related areas required. Licensed Master Social Worker preferred.
- Certified School Social Worker or eligible for certification preferred.
- Minimum of 2 years of successful experience with at-risk youth and/or youth with special education and mental health needs preferred.
- Relevant licensure.
- Must be available to work a 40-hour work week.
- Must be able to lift 25 pounds.
- Must be able to stand and run for moderate periods of time.
- Must have a valid NYS driver's license.
- Must be able to perform restraints and maintains TCI certification.
- Willingness to respond to the needs of a culturally probation.
