Overview
Varahe Health Services is seeking a compassionate and skilled Palliative Care and Geriatric Medicine Nurse Practitioner to join our dynamic healthcare team. You will work closely with patients, families, and interdisciplinary teams to ensure holistic care tailored to individual needs. Responsibilities
- Conduct thorough assessments and diagnostic evaluations of patients with complex medical conditions.
- Develop and implement personalized care plans that address physical, emotional, and spiritual needs.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and discharge planning.
- Provide education and support to patients and families regarding disease processes, treatment options, palliative care and geriatric principles.
- Manage symptoms related to serious illnesses

Qualifications
- Master’s degree in Nursing with a focus on Palliative Care or Geriatric Medicine is a plus.
- Current Nurse Practitioner licensure
- New graduates welcome!
- Strong understanding of geriatric care principles and dementia-related challenges.
- Excellent communication skills with the ability to work collaboratively within a multidisciplinary team.
