HOME HEALTH LVN

• Varco Healthcare is looking for a caring LVN to add to our team. This experienced LVN will be a great communicator, with an eye for detail and a great work ethic. Our expectations for this position are as follows:

Responsibilities:

• Assists in identifying the patient’s physical, psycho-social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations
• Participates in planning and implementing care in conjunction with the RN, in according with the plan of care
• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
• Assure the continuity of care through delivery of quality patient care
• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations
• Monitor assigned cases to ensure compliance with requirements of state, federal, and hospice policies and procedures

Job Types: Full-time, Part-time, PRN

Pay: $35.00 - $45.00 per hour
