Job Overview :
Our client a 102bed Skilled Nursing Facility in Gallup NM is looking for an experienced Occupational Therapist (OT) to join their team. This is a fulltime permanent position offering a Monday to Friday schedule with no weekend rotation. As an OT you will play a key role in helping patients regain or improve their ability to perform activities of daily living (ADLs) and enhance their overall independence and quality of life.
Key Responsibilities :
- Conduct assessments of patients physical mental and emotional capabilities to identify functional limitations and develop appropriate treatment plans.
- Provide individualized occupational therapy services including exercises adaptive equipment training and techniques to improve functional independence in daily activities.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Educate patients and families on strategies for improving daily living skills and using adaptive equipment.
- Maintain accurate uptodate patient records documenting treatment plans progress and any changes in condition.
- Collaborate with other healthcare professionals including physical therapists nurses and physicians to coordinate care and ensure comprehensive treatment.
- Ensure that therapy environments and sessions are safe following all safety and infection control protocols.
- Participate in team meetings to review patient progress discuss treatment plans and make necessary adjustments.
