Job Title: Adult Foster Care Program Director
Job Summary:
The Adult Foster Care (AFC) Program Director oversees the day-to-day operations of the AFC program, ensuring compliance with state regulations, delivering high-quality care to participants, and supporting caregivers. The manager collaborates with staff, caregivers, and external agencies to ensure the program meets the needs of the participants and maintains operational excellence.
Key Responsibilities:
- Program Oversight:
- Manage all aspects of the AFC program by Massachusets regulations, e.g., MassHealth requirements.
- Develop and implement policies and procedures to ensure compliance and quality care.
- Monitor and evaluate program effectiveness, making adjustments as needed.
- Staff Supervision:
- Recruit, train, and supervise AFC staff, including case managers, nurses, and administrative personnel.
- Conduct regular staff meetings and performance evaluations.
- Provide ongoing support, training, and development opportunities for team members.
- Participant and Caregiver Support:
- Oversee the assessment and enrollment process for new participants.
- Ensure caregivers receive adequate training, support, and resources.
- Address participant and caregiver concerns promptly and effectively.
- Compliance and Documentation:
- Maintain compliance with all regulatory and funding requirements.
- Oversee accurate and timely documentation, including participant care plans, assessments, and progress notes.
- Prepare for audits and inspections by state agencies.
- Financial Management:
- Develop and monitor program budgets in collaboration with the finance department.
- Ensure accurate billing and reimbursement processes.
- Identify opportunities for program funding and growth.
- Community Engagement:
- Foster relationships with community organizations, healthcare providers, and other stakeholders.
- Represent the AFC program at community events and meetings.
- Advocate for the program and its participants within the community.
Qualifications:
- Education: Bachelor’s degree in social work, nursing, healthcare management, or a related field.
- Experience:
- Minimum of 3–5 years in a supervisory role within human services, healthcare, or a related field.
- Experience managing Adult Foster Care programs or similar services is highly desirable.
- Skills:
- Strong leadership and interpersonal skills.
- Excellent organizational and time management abilities.
- Knowledge of state and federal regulations governing AFC programs.
- Proficiency in electronic health records (EHR).
- Conflict resolution and problem-solving expertise.
Work Environment:
- This position may require travel to participant homes, caregiver locations, and community events.
