SUMMARY OF POSITION:
The Day Habilitation Licensed Practical Nurse (LPN) reports to the RN Health Care Supervisor for Day Hab. The LPN will be responsible for implementing ROM routines and documenting in accordance with the plans. The LPN will also be responsible for all duties as assigned by the RN Health Care Supervisor. This position will be compensated on an hourly basis. This position will require some flexibility based upon program needs. May be asked to support other Day Programs with travel up to 50 miles per day.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assessments specific to each person’s health care needs.
- Administration of medications as assigned.
- Write monthly progress notes and weekly nursing assessments.
- Notify the Health Care Provider and Director of Nursing of any medical changes or concerns then communicate and document all information.
- Communicate any discharge instructions, treatments or medication changes to all staff.
- Physically assist individuals in transfer and carries, both ambulatory and non-ambulatory.
- Physically assist persons in personal hygiene tasks which may include lifts and physical support to persons with body weights of 150 - 250 pounds.
- Educate and assist staff and individuals with nutrition and dietary needs.
- Follow agency policies and procedures as defined in the agency’s Policy and Procedure Manual.
- Identify safety risks and possible alternatives and advocate for the rights of all individuals.
- Maintain confidentiality in keeping with HIPAA regulations.
- Handle related job duties in compliance with agency’s Programmatic Policy Manual and Medicaid regulations.
- Responsible for implementing ROM routines and documenting in accordance with the plans.
MARGINAL DUTIES AND RESPONSIBILITIES:
- Attend and participate in meetings, trainings and development.
- Actively participate on work teams to solve problems, share ideas and plan for change.
- Maintain confidentiality and positive demeanor while successfully juggling multiple priorities.
- Provide recommendations to your supervisor and Director of Nursing regarding ways to improve the current systems
- Maintain CPR & AED certifications.
- Maintain Safety Care certification, if required by position/program.
- Other duties as assigned.
PHYSICAL AND MENTAL REQUIREMENTS:
- Complete Agency Orientation.
- Ability to establish rapport with the disabled population, their families, co-workers and service providers.
- Maintain trainings specific to individual programmatic issues.
- Ability to plan and organize your work and the activities of others.
- Actions should reflect judgment that would not jeopardize the health and safety of the individuals.
- Proficient in Microsoft Office and a working knowledge of office equipment.
- Effective verbal and written communication skills
- Strong interpersonal and communication skills, and the ability to work with a wide range of employees.
- Must be able to bend, lift, stoop, reach and file.
Requirements
MINIMUM ACCEPTABLE QUALIFICATIONS:
- Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
- At least one year of full time or equivalent part time experience as an LPN.
- Must be 21 years old, have a valid driver’s license, daily use of a car on site and be available by telephone for daily contact.
