The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) role for Veracity Onsite will be responsible for providing total health care management for an employer-based population.

This Advanced Practice Provider (Nurse Practitioner or Physician Assistant) must be committed to putting the patient first by understanding the total person, their health risks, their goals and objectives and work collaboratively with the patient to engage them in maintaining or improving their health.

Responsibilities:

1. Strategic Health Care Planning and Alignment

a. Providing primary care and oversight for the employer-based population by reviewing all health risk factors gained through a health risk assessment, physical and evaluation.

b. Provide input and counsel to benefits design, incentive design, health care culture change efforts, etc.

2. Health Care Delivery

a. Conducting annual health risk assessments on all employees and adult dependents and developing with the patient tailored action plans

b. Providing medical services to an employer-based population, responding to questions and concerns of employees.

c. Understanding and implementing medical protocols and clinical standards.

d. Developing, overseeing and direction support staff of the clinic

e. Developing relationships with the community of providers to oversee all the clinical health of the employees

f. Oversee clinical delivery of care, specifically including supervision of the nurses, nurse practitioners, physician assistants and medical assistants at the Veracity Clinic

g. Utilize technology to ensure efficient management of patients

h. Recommend changes in all clinical areas and/or in general areas to improve patient health and wellness

i. Manage chronic diseases with highly interactive care to facilitate successful patient outcomes (education, nurse support and tools provided)

3. Support patients in changing health-related behavior

a. Serving as an educational provider for clinical support staff and employees

b. Establish goals for each patient to be monitored and tracked by support staff

c. Support patients in changing health related behaviors (education, nurse support and tools provided)

d. Participate in patient education sessions among targeted populations with prevalent health risk factors, such as high body mass index, smoking, diabetes, and diet)

• Medical office experience and patient care expertise
• Proficient in basic life support and patient assessment
• Knowledge of medical terminology and HIPAA regulations
• Experience administering injections and conducting physical examinations
• Familiarity with EMR systems such as Epic and Cerner
• Critical care experience and skills in triage and acute pain management
• Certification in health coaching and relevant clinical research experience
