Case Manager Needed - M-F
Compensation - TBD upon experience
Scheduled Hours - M-F, approximately 8:30-5pm.

Currently, The Center at Val Vista is looking for Case Manager - LPN or RN to join our Team. This fast paced position requires a positive team member who can work well under pressure and is comfortable with multi-tasking.
Job Duties and Responsibilities include but are not limited too:
- Communicate resident status, change in function, and careplan
- include payer representative in interdisciplinary meetings
- document all payer interactions
- maximize benefits by coordination
- Participate in all IDT meetings
- Assist in coordinating/planning discharge plans
- Assist in communication to physicians, with transcription of orders, documentation process, transfer/admission, and or DC process
- Ensure appropriate supplies/equipment are available to meet the needs of the assigned patients.
- Maintain confidentiality
- Provide data to Quality Assurance Committee as requested

Qualifications
Must be able to pass a pre-employment background check/drug screen
Must have a valid level 1 fingerprint clearance card
Must have a valid CPR Certification from an In-Person Class only.
Must have valid and in good-standing LPN or RN license in the state of AZ
