The Center at Lincoln is looking for a Part Time Case Management Assistant Wednesdays to Fridays. The Case Management Assistant's job is to support clients and to assist the Case Managers in helping people receive the treatments and resources they need.
Case Management Assistant responsibilities include:
- Complete Home Health referrals based on Physician’s orders by obtaining orders and calling Home Health agencies.
- Complete Durable Medical Equipment (DME) referrals by assembling packet of information, faxing documents and calling DME companies.
- Maintain confidentiality of necessary information and to adhere to HIPPA.
- Assist in completion of referral packets by assembling packet of information and faxing documents.
- Assist in completion of community referral applications e.g., In-Home Supportive Services and Home Health Care Services
- Other duties as assigned by Case Management
Qualifications
Qualifications, Skills, Abilities:
- High school diploma or equivalent.
- Ability to communicate effectively with patients and their families, and at all levels of the organization.
- Excellent time management skills to organize and prioritize workload.
- Strong English communication skills (written, verbal and comprehension)
- Strong interpersonal skills, proven ability to handle sensitive and confidential situations
- Strong analytical, organizational, time management and problem solving skills
- Ability to work cooperatively as a member of a team
- Demonstrated strong computer skills including MS Office, Excel, Word, and electronic health records.
Physical Demands:
- Ability to stand and/or sit for extended periods
- Fine motor skills
- Good visual and auditory acuity
Pay Range $23.00-$25.00
Salary : $23 - $25
