Hospice RN Case Manager

Job Summary

We are looking for an experienced registered nurse who initiates and coordinates the hospice plan of care, supervises CHHAs and LVNs and provides skilled nursing hospice and palliative care services to our patients at their place of residence. The RN will work cooperatively with the attending physician, Hospice Medical Director, and other members of the Interdisciplinary Hospice Team. The RN regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations and provides education and supportive care to patient and caregivers. Must be able to utilize computers and be comfortable with electronic medical records.

Responsibilities

• Conduct initial and on-going comprehensive assessments of patients to determine their healthcare needs and develop individualized care plans.
• Collaborate with interdisciplinary team, including physicians, social workers, and other healthcare professionals, to optimize patient outcomes.
• Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
• Maintain accurate documentation of patient interactions, care plans, and progress notes in compliance with HIPAA regulations.
• Stay current with best practices in hospital medicine, emergency medicine, and medical-surgical nursing to provide high-quality care.

Qualifications

• Current Registered Nurse (RN) license in the state of California.
• Experience in case management or a related field is required.
• Strong communication skills with the ability to work collaboratively in a team environment.
• Previous experience in a clinical setting such as a hospital or clinic is highly desirable.

Job Types: Full-time, Part-time

Pay: $150,000.00 - $190,000.00 per year

Expected hours: 32 per week

• 8 hour shift
