The Care Coordinator at Veros Health plays an essential role in patient care coordination and the intake process, ensuring seamless communication between the patients, clinical intake team, and the authorization team. This role primarily focuses on infusions and injectable therapies, guiding patients through the process from initial provider order to scheduling and treatment completion. Care Coordinators act as the patient’s primary point of contact, ensuring they are informed, supported, and scheduled in a timely manner while collaborating with multiple departments to improve efficiency and patient experience. This position owns the process from start to finish, requiring strong organizational skills, efficiency, communication, and the ability to hold team members accountable.
- Manage the entire coordination process for infusions and injectables, from provider order request to treatment scheduling.
- Track and process provider orders in Greenway, ensuring timely and accurate followup.
- Contact patients within 24 hours to introduce themselves, explain next steps, and help patient with gathering all the medical records they need for approval using designated checklist.
- Monitor and manage the prior authorization process, working closely with the intake and authorization teams to ensure approvals within a expedited time frame.
- Serve as the primary liaison between patients, and internal teams to eliminate workflow bottlenecks and miscommunications.
- Ensure that clinic intake teams build and submit LMNs (Letters of Medical Necessity) promptly, tracking progress and following up as needed.
- Follow up with the billing and authorization team within 3 days of submission to verify approval or initiate an appeal if necessary.
- Oversee patient scheduling upon insurance approval, ensuring no delays in treatment start dates.
- Track denials and appeals, ensuring necessary documentation is resubmitted and working with the clinical intake team on appeal requests if needed.
- Hold team members accountable to standardized workflows and drive improvements to increase efficiency in scheduling and approvals.
- Experience in healthcare coordination, medical assisting, or patient advocacy preferred.
- Knowledge of infusion therapies, biologics, and injectable treatments is a plus.
- Strong organizational skills with the ability to manage multiple patient cases simultaneously and stay on top of details.
- Excellent communication and follow-up skills, ensuring patient engagement and provider collaboration.
- Familiarity with Greenway and WeInfuse (or other EHR and infusion management platforms) is highly preferred.
- Self-motivated, detail-oriented, and proactive in ensuring efficiency and patient satisfaction.
- Ability to problem-solve and escalate delays as necessary to ensure workflow continuity.
- Strong ability to work with prior authorization teams, intake specialists, and billing departments to drive efficiency and compliance.
