Job Description
This position reports directly to Administrator of facility. The primary responsibilities of your job position are to plan, organize, develop, and direct the overall operation of the Admissions Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator. As Director of Admissions, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. This position will act as a liaison for skilled nursing facilites and assigned hospitals and designated referral sources for families. This person will be responsible for building relationships with hospitals, and other sources which generate patient referrals and admissions. The Admission Director will assess clinical needs of potential / existing residents together with the Director of Nursing or designate person to assure the facility has the clinical capability to treat the resident.
Duties and responsibilities may include but are not limited to :
INTERNAL RESPONSIBILITIES
Manages the daily census numbers, including admissions, discharges, etc. Schedules admissions and works closely with all disciplines to prepare the for admission and their special needs. Reviews, with the applicant and / or responsible party, Medicaid, Medicare Eligibility requirements, and private pay rates before admissions. Ensures verification of primary and secondary payors. All potential admissions must be verified for active Medicaid coverage through the resident's demographic information. For all admissions with DME orders, Medication authorization, etc., the Admission Director will work alongside the DON to coordinate and facilitate timely equipment ordering before the resident arrives. For all admissions with follow-up appointments, the scheduler must receive a printed-out copy to ensure appointments are scheduled before resident admission. Updates all admissions materials and forms on an ongoing basis. Reviews PRIs, SCREENS, and applications with the admissions commitee (Interdisciplinary team). PRIs must have a valid social security number filled out for all accepted admissions. Ensure financial verification is accurate and complete before the patient is admited to the facility and communicates with the Interdisciplinary team all information for a seamless admission process. Maintains accurate information in the Sigma system, including the dashboard, assigned patient information, and document management. Verifies incoming admissions fit the scope of service provided by the facility. Participates in selecting appropriate room assignments to help facilitate the adjustment into their new setting. Reviews and evaluate remaining Medicare days to ensure days are accurate and available to bill. If there was a previous Nursing Home Stay, Admissions must identify the previous facility and contact them to get accurate lengths of stay and payor used for this specific period to account for Medicare days used and remaining accurately. This information must be shared with the Interdisciplinary team. If no secondary payor source, the resident or family must be contacted to ask if they will pay privately and or apply for Medicaid for any out-of-pocket cost, they are responsible. For all Nursing Home Transfers, obtain financial documentation to identify a source of NAMI payment to ensure timely reimbursement to the facility. A current Medicaid Budget Approval and a current PIA statement from the previous facility should accompany all nursing Home Transfers cases. Supervises, directs and manages all Admissions Department employees holding them accountable to policies, procedures, and the professional standard set forth by the Administrator. Completes other duties as assigned or necessary for the company's successful operation.
EXTERNAL RESPONSIBILITIES
Responsible for increasing the referrals from assigned primary and secondary sources, including but not limited to hospitals, home health, senior living centers, senior daycare centers, outpatient medical facilities, assisted living centers, clinics, and veteran’s associations. Responsible for the education of referral sources on the facilities bed availability and admissions processes. Communicates any barriers or challenges that occur within the referral process whether internal or external problems. Conducts pre-planned sales calls to the facilities major referral sources including hospital’s case management, assisted living, senior centers, key physicians, therapy departments, hospital networks, etc. And Quality Mix practices including orthopedics (doctors, mid-levels, practice managers), Neuro / stroke and cardiac care centers, trauma and ortho trauma centers, respiratory care centers, etc
