- 17.625 PACT Providers
- 3 GAP/SDA providers
- 1 ACOS (Assistant Chief of Staff)
- 1 Assistant ACOS
- 5 Administrative staff members
Practice Environment Description:
- Outpatient panel size: 1200 patients
- Reports directly to the Primary Care ACOS
Academic Affiliations:
- Wake Forest School of Medicine

Requirements:
- U.S. Citizenship (non-citizens may be considered if no qualified citizens can be found)
- Doctor of Medicine degree or equivalent from an accredited institution
- Unrestricted medical licensure in any U.S. state or territory
- Completion of a residency program in an accredited core specialty
- Board certification or board eligibility in Internal or Family Medicine
