Job Title: Case Manager (Homeless Veterans Reintegration Program)
Location: Vineland, NJ
Type of Position: Full-time, Non-Exempt
Reports to: Program Manager
The Case Manager ensures that participants receive essential supportive services to aide in the development and achievement of individual employment plans. In addition, the Case Manager ensures service delivery through viable resources such as employment, clothing, housing, referral to medical or substance use disorder treatments, and transportation assistance — either within the Veterans Multi-Service Center or at another agency in the community.
KEY RESPONSIBILITIES
- Consults with program applicants to assess need and determine program eligibility, ensuring that clients obtain needed services;
- Works with clients to create individualized employment plans with client-specific goals;
- Provides ongoing support and expertise through assessment, planning, implementation, and overall evaluation of the veteran’s needs as drawn from his/her goal/action plans;
- Obtains information contributing to the client’s situation and evaluating these, along with the client’s abilities and skills;
- Collaborates weekly with the Program Manager on action/service plans and coordinating approaches for successful outcomes;
- Conducts outreach to identify and provide services to target populations;
- Develops and maintains professional relationships beneficial to program and agency in order to generate program referrals and veteran placement;
- Attends all internal, external and community meetings and events relevant to the position and/or ensures staff attendance at all applicable meetings and events;
- Ensures service delivery through adherence to Department of Labor and agency requirements
QUALIFICATIONS
- Bachelor's Degree preferred; equivalent education and experience considered
- 3 years customer service, social work or non-profit experience required
- Basic Microsoft operating system navigation, Office, Outlook, Word, Excel, Power-Point and Internet Browser
- Experience working with U.S. Armed Forces veterans a plus
- Excellent communication skills; ability to work independently with positive results
- Valid driver's license with reliable registered and insured automobile
