Title: Veteran Peer Support Specialist – SSG Fox SPGP
Position Summary:
A Full-time position of Veteran supporting Veterans and Service Members, during times of need and specifically those in a mental health crisis. Providing support to these individuals by strengthening their connectivity with community resources that the peer support specialist will be an expert in. Primary duties will be crisis response and outreach to our most vulnerable Veterans population. This will also include intensive peer support to Veterans and their families including the homeless community across NC, targeting Veterans who may be in crisis and may suffer with a serious mental illness or co-occurring disorders of such severity that the impairment substantially interferes with major life activities. Provide immediate support within 24 hours of notification of potential mental health crisis. Provide peer support to these individuals, as he or she progresses in recovery, independence, self-sufficiency goals or stabilizing health (behavioral or medical), through behavioral health services, community medical providers and local resources through a continuum of care.
Position Type:
Classification –Full-time salary position with flexible hours, including occasional evenings and weekends.

Duties / Responsibilities:
· The Peer Support Specialist will be responsible for extensive outreach to Veterans and will partner with other outreach programs such as HOPE and PATH.
· Will administer the COLUMBIA-SUICIDE SEVERITY RATING SCALE (C-SSRS), where applicable.
· Provide support and linkage to community resources in an attempt to prevent future crisis and will utilizing both NCServes and NCCare 360.
· Employ the embedded C-SSRS in the Unite Us Platform, at each encounter with Veterans to determine their level of risk for suicide.
· Conducts biopsychosocial assessments in person with clients to identify their needs in key areas (e.g., mental health, substance abuse, housing, employment, financial assistance, benefits, social connection, etc.).
· Consults with Coordination Center staff to make referrals (Utilizing the UniteUs Platform) to Network providers.
· Addresses identified needs based on understanding of eligibility criteria, capacity, location, and client preference within the spirit of the Peer Support model.
· Maintain a working knowledge of Network providers’ menu of services, eligibility criteria, intake process and preferences, specifically in MH/SU systems.
· Facilitates community integration and continuity of care by ensuring that the individual receiving services has enduring ties to their community and support systems particularly while in crisis or pre-crisis situations.
· Participate with multiple case management teams to support the member and integrated care team’s goals in a holistic approach.
· Provides Peer Support with clients to assure proper services and needs are being addressed at appropriate times.
· Documents all activity in accordance with grant program guidelines and in compliance with internal policies and procedures.
· Participates in Outreach efforts for and represents ABCCM in a professional manner at appropriate meetings, functions and events.
· Coordinates across other programs and ministries within ABCCM.
· Participates in all appropriate staff, supervision, and training meetings. Assumes other duties as assigned by Senior Management.

· Proficient with the Columbia Suicide Severity Rating Scale (C-SSRS)
· Proven ability to perform at a high level in a fast-paced, changing environment.
· Experience working with veterans.
· Must have knowledge of clinical issues, skill in community resource development, be well organized, a self-starter, and able to work as a member of a team.
Relationships / Working Environment:
· Coordinate and partner with law enforcement and first responders; as well as local mental health providers, crisis centers, and churches.
· Must be willing to be available 24/7 for crisis response for a Veteran struggling with mental health.
· Prepared to conduct outreach in potentially dangerous areas and potentially harsh conditions.
Qualifications:
- Must be a Veteran as defined in USC Code.
- Must be a NC Certified Peer Support Specialist (within 6 months of Hire)
- High School Diploma or equivalency is required for this position but preference will be given to BS in Human Services.
- Lived experience with MH, SU or I/DD issues with receipt of services and supports as a consumer or family member is desired. Existing knowledge of the area resources available to individuals preferred.

Pay: $19.00 - $21.00 per hour
