The Steven A. Cohen Military Family Clinic at VVSD provides high-quality mental healthcare services accessible to all post-9/11 (including National Guard and Reserves), their families, and the families of active-duty service members including spouse or partner, children, parents, siblings, caregivers, and others.
Compensation: $68,000-$74,000 salary
Job Summary:
Provides support to the Cohen Clinic as the first contact for services. The Intake Coordinator will conduct phone and face to face screenings and intakes on-site, via telehealth, and off-site (if applicable) for veterans, military, and their families. The Intake Coordinator will work with a multi-disciplinary team for case assignments, consultations, and care coordination.
Essential Duties and Responsibilities:
• Conduct referral screeners with veterans and /or family members calling or walking into the clinic to determine if they qualify for services.
• Conduct Biopsychosocial Intake as needed to provide quicker access to services.
• Performs and documents client’s screenings, biopsychosocial assessments, progress and referral information.
• Meet regularly with Clinic Director and/or Lead Clinician to coordinate and consult regarding cases.
• Track and report on referral to admission conversion rate.
• Knowledge of local community resources and provide warm hand-offs for veterans and families needing external referrals.
• Work closely with the Business Office Manager to ensure billing requirements are adhered to.
• Provide on-going program evaluation and recommendations for continuous growth and quality.
• Attend weekly clinical team meetings
• Provide services via telehealth platform, as required.
Qualifications

Qualifications:
• Master’s Degree in Social Services field. Active ASW, AMFT, APCC, or equivalent. Maintain Board Registration /or certifications as required by state regulations. Stay current in the field of mental health, counseling, case management and other related social services.
• Minimum of 3 years’ post master’s intake coordination and/or case management preferred. Experience working with Veteran and Military population highly preferred. Bilingual candidates are highly preferred.
• Must maintain regular and acceptable attendance.
• Working knowledge of MS Word, Outlook and navigation of an electronic health record system.
• Knowledge of mental illness and mental health diagnoses.
• Knowledge of various treatment modalities and methods.
• Exhibit good judgment.
• Ability to communicate clearly and effectively via oral or written means.
• Ability to present a professional and positive demeanor to staff, funders, officials and the general public.
• Ability to remain calm and maintain self-control amid difficult circumstances and emergencies.
• Ability to think analytically and evaluate the impact of treatment options and recommendations.
• Ability to set work priorities and to evaluate and create solutions to work related problems.
Physical Requirements:
• Ability to bend and lift light objects

Salary : $68,000 - $70,000
