This full-time position (MondayFriday, 40 hours per week) is for the primary on-site provider at a rehab facility, focusing on skilled nursing and post-hospital care. Responsibilities include providing primary care services to patients in rehabilitation. A medical director will be on-site one day per week, with support from LPNs and CNAs. All calls are managed by a triage service.
