The Clinical Coordinator has the opportunity to positively interact with patients, family members and team members daily. Home Health Nurses are passionate about providing the highest quality care possible and exceeding the expectations of those they serve and work with.

Our Home Health Clinical Coordinator would work closely with the Administrator and intake team to review eligibility of referrals to Home Health. The successful candidate would be knowledgeable in OASIS, PDGM, and Home Health regulations to provide education to staff on these to ensure compliance within them.

Qualified candidates will be either a Registered Nurse, Physical Therapist, Occupational Therapist, or a Speech Language Pathologist. They will possess high personal integrity, a caring attitude and portray a positive image. They must also have a current CPR certification.
