LTC Community Team Lead
Job Description
The LTC Community Team Lead is responsible for leading a team of healthcare professionals in providing care and support to residents in a long-term care community. The Team Lead will work closely with the Director of Nursing and other members of the healthcare team to ensure that residents receive high-quality care that meets their physical, emotional, and social needs.
Responsibilities:
- Lead and manage a team of healthcare professionals, including nurses, nursing assistants, and other support staff
- Develop and implement care plans for residents that address their physical, emotional, and social needs
- Ensure that all care provided meets regulatory requirements and industry standards
- Collaborate with the Director of Nursing and other members of the healthcare team to ensure that residents receive comprehensive care
- Provide guidance and support to team members, including training and development opportunities
- Monitor and evaluate the performance of team members, providing feedback and coaching as needed
- Communicate regularly with residents and their families to ensure that their needs and concerns are being addressed
- Participate in quality improvement initiatives and other projects as assigned
Requirements:
- Current nursing license in good standing
- Minimum of 3 years of experience in long-term care or a related field
- Strong leadership and management skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively with other members of the healthcare team
- Strong problem-solving and critical thinking skills
- Knowledge of regulatory requirements and industry standards for long-term care
