Travel: 75% travel to the Bronx, Manhattan, Queens, and Brooklyn
Sched ule: Monday- Friday - 9-5; flexible to some evening hours to fit client availability
Hourly Pay: $28.57-29.18

Position Overview:
Child & Adolescent Health Home Care Coordination is staffed with dynamic, youth health advocates. The aim of care coordination is to build well-working teams by ensuring that all parties are informed and effectively communicating, while also linking services that could further help a young person to achieve the best possible outcomes. The team's approach to Care Coordination is oriented towards wellness, is family-focused, and supports a strength based, person-centered system of care. Care Managers ensure that the children and adolescents receive timely access to services, as well as, coordination and management of care across all systems: education, mental health, juvenile justice, child to adult.
