JOB SUMMARY
We are seeking a dedicated and compassionate Private Duty Home Health Nurse (RN or LPN) to provide personalized, one-on-one care to clients in their homes. The ideal candidate will deliver skilled nursing services with a focus on safety, dignity, and patient-centered care. This position reports directly to the Director of Home Health and plays a key role in promoting the highest standards of clinical excellence.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Deliver direct, individualized nursing care in accordance with the patients care plan
- Administer medications, treatments, and injections as prescribed
- Provide weekly medication box set-up
- Monitor vital signs and overall health status, reporting significant changes promptly
- Perform wound care, catheter management, and other skilled nursing tasks as needed
- Educate patients and their families on disease management, medications, and home care techniques
- Collaborate with the interdisciplinary team, including physicians, therapists, and home health aides
- Accurately document all care provided, maintaining compliance with agency policies and state regulations
- Maintain a clean, safe, and supportive environment in the home setting
JOB REQUIREMENTS
- Active and unrestricted RN or LPN license in the state of Florida
- Current CPR/BLS certification
- Minimum of [12] years of nursing experience; home health or private duty experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication, time management, and organizational skills
- Ability to work independently and adapt to changing patient needs
PHYSICAL DEMANDS
While performing the essential functions of this job, the employee is regularly required to stand, walk, sit, talk hear, and occasionally required to reach with hands and arms, stoop, kneel, and crouch. The employee must occasionally lift and/or move up to 30 pounds and be able to work at heights. Specific vision abilities required by the job include close, distance, and peripheral vision, depth perception and the ability to adjust focus as well as work on a computer for extended periods of time. The employee must be able to communicate verbally with others on the phone or in person.
