Job Summary :
Under the direct supervision of the assigned Supervisor, the Family Support Counselor has responsibility for providing support services to individuals and families as identified in the designated program and / or individualized plan.
Program Info :
Wraparound is a high touch / high intensity program that provides a family-centered, strength-based, needs-driven planning process that determines the individualized services necessary for children, youth, and families to live safely in family settings and avoid out of home placement. Services / interventions are specific to each family but may include family finding, child and family teaming, care coordination, and mental health services for children and / or their family.
