As a key member of our team, you will be instrumental in delivering compassionate, personalized care that improves the quality of life for our residents and their families.
Key Responsibilities:
- Conduct in-depth interviews with residents and families to gather comprehensive social histories, ensuring care plans are tailored to meet individual needs.
- Collaborate with residents, families, and the care team to design and implement customized social service programs that address social, psychological, and emotional needs.
- Identify and resolve social and environmental challenges, offering resources such as financial assistance, discharge planning, and connections to community organizations for specialized services.
- Partner with administration, nursing teams, and other departments to develop and execute engaging activities and comprehensive social service programs.
- Actively participate in surveys and inspections conducted by government agencies, ensuring the facility remains compliant with quality standards and regulations.
- Serve as a passionate advocate for residents by communicating their needs and coordinating with physicians and medical professionals to ensure optimal care.
Requirements:
- Minimum of 3 years of experience in social services, ideally within a skilled nursing facility (SNF).
- Exceptional interpersonal, communication, and problem-solving skills.
- A strong commitment to providing excellent care and advocating for the emotional and social well-being of residents.
