Key Responsibilities :
- Conduct new and re-admission assessments.
- Perform quarterly, annual, and significant change assessments.
- Monitor and document residents' weights, weight loss, pressure ulcers, and wounds.
- Oversee residents undergoing hemodialysis and enteral feeding.
- Complete MDS (Minimum Data Set) documentation.
- Update care plans and participate in care plan meetings.
- Update meal and snack preferences, enter diets, and assist with menu reviews.
- Conduct meal rounds and audits as assigned.
- Provide nutrition education and counseling to residents and their families.
- Attend morning reports as needed.
- Maintain effective communication with the medical team and interdisciplinary staff.
