What are the responsibilities and job description for the Master's level Co-Occurring Counselor position at Victory Clinical Services IV?
· Provides counseling and therapy services to individuals and families who have drug related problems and/or co-occurring problems.
· Counsels patients on an individual and group basis; attempts to aid patients through contact and case coordination with other community resources.
· Orients patients to treatment.
· Maintains timely and accurate documentation of services provided.
· Participates in quality improvement activities.
SPECIFIC DUTIES[1]:
· Provides evidence-based counseling and therapy on a one-to-one basis, as well as group and family counseling. A minimum of 50% of employee’s time will be spent in direct services with patients and/or families (spending 60% of one’s time in direct services will results in a bonus);
· Provides counseling services to patients with co-occurring disorders;
· Refers patients to appropriate service agencies in either the immediate community or surrounding areas;
· Enters or requests authorization for services from the patient’s funding source based on clinical and medical necessity;
· Attends staff and treatment meetings;
· Attends workshops and in-service training sessions on substance abuse;
· Provides emergency crisis intervention as needed whether onsite or on the telephone;
· Handles all patient information in a confidential manner;
· Completes intakes, narratives, person-centered plans and any other necessary paperwork according to the policies and procedures of VCS;
· Thoroughly explains program rules, rights, guidelines and expectations to patients during orientation and throughout treatment;
· Effectively articulates to patients the consequences of non- compliance with signed program rules;
· Manages caseload;
· Develops and maintains one-to-one counseling with each patient in a manner convenient to both patient and counselor;
· Develops a person-centered plan (treatment plan) with each patient in a manner understandable to both patient and counselor. Reviews the person-centered plan according to VCS policies;
· Completes releases of information with patients, upon their request, so we may communicate with sources outside of the clinic regarding their treatment;
· Completes 60-day case reviews;
· Assists in peer to peer case review;
· Completes the transition plan and discharge summary for patients; and
· Performs other related duties as assigned.
