Job Summary:
As the HSE Coordinator at Victory Human Services, you will play a vital role in assisting individuals in identifying, accessing, and maintaining suitable housing. Utilizing a recovery-oriented and person-centered approach, you will work closely with everyone to expedite housing support and ensure their housing needs are met.
Key Responsibilities:
· Develop and maintain relationships with local property managers, landlords, and housing authorities to facilitate housing opportunities for individuals.
· Conduct outreach services to identify housing needs and preferences of individuals currently residing in various environments, including shelters, temporary housing, and residential settings.
· Assist individuals throughout the process of securing and maintaining housing, providing support and guidance as needed.
· Collaborate with internal and external stakeholders to address barriers to housing and advocate for the housing rights of served individuals.
· Maintain accurate documentation of housing-related activities, including progress notes and housing plans.
· Coordinate appointments and transportation for individuals to obtain essential documentation and services.
· Provide supportive counseling to address personal and social issues related to housing stability.
· Assist with housing subsidy programs and affordable housing options, ensuring accessibility.
Requirements
Qualifications:
· High school diploma or equivalent required; bachelor's degree in mental health, social services, or business preferred.
· 1-2 years of experience working with individuals with medical conditions/mental health concerns preferred.
· Experience managing affordable housing or housing support is desirable.
· Knowledge of housing subsidy programs and real estate brokerage or management is advantageous.
· Must have access to an operational and insured vehicle for transportation.
