Position Summary
- Deliver direct, quality patient care, utilizing a basic clinical skill level and knowledge base. Develop, implements and evaluates / revises the interdisciplinary plan of care.
- Facilitates and coordinates best practice standards for Transplant and Living Donor patients throughout their transplant journey
- Manages all aspects living donor evaluations and transplant recipient evaluations.
- Provides direct patient care to post transplant patients in the clinic and coordinates their post operative care.
- Provides patient education for all patients seen in the transplant clinics and / or their primary caregivers.
- Participates in Research / Quality Improvement and patient safety initiatives for the living donor and transplant population.
