Required Qualifications:
- Bachelor's degree in Social Work (BSW)
- Experience as a Social Worker in a Skilled Nursing Facility
- 23 years of management experience, with at least two years of supervised social work in a healthcare setting
- Proficiency in completing the Uniform Assessment Instrument (UAI)
Key Responsibilities:
- Lead the Social Services Department, ensuring all services align with resident needs and are in compliance with state and federal regulations
- Complete and delegate the social services component of the comprehensive assessment, creating individualized care plans that promote well-being and support mental and psychosocial needs
- Oversee preadmission services, including participation in interdisciplinary evaluations and PASARR Level I completion
- Ensure residents are safely transitioned back into the community, arranging for follow-up care and coordinating community-based services
- Develop and maintain a thorough documentation system that meets legal, ethical, and professional standards
- Ensure high-quality care through the establishment and oversight of departmental quality assurance (QA) procedures
- Oversee Advance Care Planning and regularly review advance directives, ensuring that end-of-life preferences are communicated and respected
- Address and resolve resident grievances and complaints, tracking trends and ensuring that all concerns are managed in accordance with facility policies
- Contribute to the development of policies that positively impact the quality of care provided to residents
- Advocate for residents rights, ensuring staff members uphold resident dignity and address allegations of abuse, neglect, or exploitation
- Lead investigations into suspected abuse, neglect, or exploitation in collaboration with the Administrator and appropriate agencies
