Job Title: Licensed Behavioral Health Clinician (Social Worker)

Shift: (40 hours per week on average)
Job Hours: M-F 8:30am-5:00pm
Base Location: Lawrenceville, Ga. - Stone Mountain St. 
Division/Department/Program: Community Courts-AD/Court Services

 

The Behavioral Health Clinician/Social Worker 2 provides professional therapeutic services to persons with mental illness and/or addictive disease.  Arranges appointment schedule at optimal intervals to maximize use of work time. Conducts face-to-face comprehensive assessment to identify problems and service needs.  Writes individual service plans that meet required standards for diagnoses, content, justifications for treatment, evidence of consumer input, and treatment time frames. Writes service plans that are clear, concise, complete, grammatically correct, timely, and properly coded. 

 

Provides individual, family or group therapeutic counseling and/or educational and skill development sessions as indicated in the service plan.  Monitors consumer progress and compliance with the service plan.  Writes timely progress notes that relate directly to the treatment outcomes described in the service plan.  Assembles, analyzes, and interprets new/updated information about the consumer in order to reassess treatment needs.  Identifies unmet needs, reviews available resources, and modifies the treatment plan as indicated.  

 

Provides crisis intervention services to existing and walk-in consumers as needed

 

May serve as case manager to facilitate and ensure communication, linkage, and continuity of care between consumers, physicians, other organizational components, community resources and service providers.   Establishes linkages and coordination of services with community agencies.  Develops and maintains cooperative working relationships with community service providers. Informs consumers and families of the availability of community advocacy and support groups.

 

Duties & Responsibilities:       

  • Conduct Screening for mental health/substance use disorder with custodial parent support and consent; administers C & A Comprehensive Assessment or Adolescent Addiction Severity Index or other instrument; formulates reports and recommendation letters for the court.
  • Assist the court and collaborative agencies in responding to youth-affected crisis situations.
  • Provide service coordination for applicable youth and their families to community resources and provide documented follow-up.
  • Research available services and programs and provide referrals for mental health, substance use treatment, and other social services. Maintain database of applicable and accessible resources.
  • Attend required training of the Gwinnett Juvenile Court.
  • Coordinate and lead the Pre-petition Multi Agency Committee (PMAC) meetings. Maintain a PMAC spreadsheet to suggest psychiatric evaluation inquires/completion, mental health assessments, and PMAC referrals, along with status updates.
  • Completes quarterly reports for the Comprehensive Services planning and PMAC meetings.
  • Conduct group, individual, and family counseling with clients in a Family Treatment Court program.
  • Attends training for various evidence-based practices and provides these treatments with fidelity to the model/s. Writes timely progress notes for all services provided.
  • Function as a member of the Family Treatment Court team.
  • Attend Family Treatment Court staffing and court sessions on a bi-weekly basis. Communicates with the Family Treatment Court team and keeps them updated on client progress.
  • Participates in treatment team meetings.
  • Provides case management to clients and links them with resources if needed.
  • Develops treatment plans and updates as needed.
  • Enter notes into court case management system.
  • Other duties as assigned

Minimum Qualifications:

Master’s degree in social work or a closely related field AND licensure in the State of Georgia as professional counselor, social worker, or marriage and family therapist at least at the associate level (LAPC, LMSW, or LAMFT).

 

Preferred Qualifications:  

  • Experience working with children, youth, and their families
  • Experience conducting Mental Health/Substance Use Disorder assessments
  • Experience working with clients involved in the criminal justice system/accountability courts
  • Experience working with clients with Substance Use Disorders/Co-occurring Disorders
  • Fully licensed in Georgia as an LPC, LCSW, or LMFT  

Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.
  • Other duties as assigned.
