What are the responsibilities and job description for the Nurse Practitioner position at Vigilant Health?
Part-Time Nurse Practitioner:
Individual will need at least 3 years of experience to lead the multi-disciplinary care team in appropriate patient care for Vigilant Health. Provide quality patient care and patient outcomes in a team environment, while assuring the standards of performance are upheld. Participate in training and development efforts of clinical staff to assure the appropriate management of plan members and the execution of well-being programming that is developed specifically for each client organization. The description below is intended to depict the general content and requirements for the performance of this position. It is not to be construed as an exhaustive statement of duties, responsibilities or requirements. Current hours of operation are Monday - Tuesday, 9:00am - 6:00pm and Wednesday 8:00am - 12:00pm. No call or weekends.
1. Provide medical patient care by interviewing, examining, and treating patients in order to meet their medical needs following established Vigilant Health protocols.
2. Educate patients on wellness, prevention and early detection by providing materials and resources to the patients and families.
3. Determine level of urgency of follow-up, referral/ consultation appointments.
4. Prescribe appropriate medical treatment and medications as outlined by Vigilant Health’s VP Clinical Operations and Chief Medical Officer.
5. Demonstrate the effective management of the plan member population.
6. Assure that appropriate provider coverage is available during clinic hours.
7. Document all services timely in patient medical record for accuracy.
8. Assure integration of well-being programs as available with each client.
9. Consult with clinic physician as needed regarding patient care, assessment, and education issues.
10. Assist in resolution of complaints, and inquiries from patients.
11. Maintain confidentiality of all patient information according to federal guidelines and regulations.
12. Works with a level of autonomy.
13. Support clinic operations by supervising staff and providing other services as needed Master’s degree in nursing Current DEA and National Certification as a Nurse Practitioner.
