To assess all individuals booked into the Vigo County Jail to identify any risks and or needs they may have. To be responsible for identifying needs for these individuals and making a plan for them upon re-entry. Must have the ability to work within a secure jail.

POSITION REQUIREMENTS:
- Minimum High School Diploma or a GED
- Minimum of 1 year experience working in the substance abuse and/or mental health field
- Knowledge of resources to connect client with basic needs including, but not limited to; food, housing, transit, health, education, work and legal.
- Ability to work with inmates in a secure jail environment.
- Must Possess valid Indiana driver’s license and demonstrate a safe driving record. Provide proof to supervisor annually and must notify supervisor of suspensions or restrictions.
- Ability to meet all department hiring requirements, including passage of a drug test.
ESSENTIAL JOB FUNCTIONS:
- Meet with inmates at the jail facility to assess need for community based resources services upon reentry
- Develops a transition plan with identified inmates
- Assists inmates in utilizing and/or applying for services provided through various state and federal agencies and local service providers.
- Facilitates and identifies additional assessments necessary for the benefit of the inmate
- Refers inmates as needed to mental health/substance abuse services, community programming, basic needs resources, and other similar programs and services
- Follow up on inmate’s progress upon reentry by maintaining open communication with the inmate and service providers
- Establishes and maintains effective working relationships with inmates, jail staff, probation/parole officers, law enforcement, courts, community providers, co-workers, and supervisors
- Acts as an advocate for the needs, rights, and interests of inmates
- Demonstrates positive beliefs and feelings regarding participants with varying levels of needs
- Effective written and verbal communication, including ability to communicate in a professional manner
- Maintain confidentiality and standards of ethical practice
- Other duties as assigned

REPORTING REQUIREMENTS:
- Provide quarterly report of clients served, caseload report and participant success story to the Vigo County Board of Health.
RESPONSIBILITIES:
- Maintain a confidential and trustworthy relationship with participants
- Represents the Vigo County Health Department in the most professional way especially when in the community
- Attend and actively participate in all staff meetings
- Maintain regular contact with all Vigo County Health Department staff
- Share news, information, new sources with staff, especially as it pertains to development.
- Dress, speak and conduct yourself in an appropriate and professional manner at all times.
ESSENTIAL KNOWLEDGE, EXPERIENCE AND ABILITY:
- Skill and ability to operate standard office equipment.
- Skill in utilizing computers, word processing and spreadsheet software, and database to perform input and retrieval functions to include Microsoft Word, and Excel.
- Ability to quickly analyze, retain composure, and utilize sound independent judgement and actions in stressful and emergency situations.
- Ability to work independently, prioritize work, meet deadlines, make decisions, and solve practical problems.
TRAINING:
- Attend relevant trainings as necessary.
- Remain educated about service agencies in Vigo County.
- Participate in development when requested.

Must understand the role of employee’s individual position regarding HIPAA requirements and level of access to Protected Health Information as defined in HIPAA hospital-wide policies.
