POSITION GRADE : 79 [ salary range : 23hr-27hr commensurate with education and experience]

SUPERVISOR : Nurse Manager; on shift may report to or Lead Nurse
JOB SUMMARY :
Under the general direction of the Nurse Manager, the Licensed Practical Nurse is responsible to promote the health and wellness of all the clients served at Living Hope Detox, and provide the care requirements in accordance with regulating bodies. The LPN is a licensed nurse whose duties will include the following : perform nursing assessments within NYS scope of practice on new intakes, obtain health and psychiatric records, complete communicable risk assessments, obtain vital signs, heights, weights and waist circumference as applicable, manage the providers’ schedules, order and administer medications, accept pharmacy deliveries, and perform any duties, within the scope of their license, requested by the RN. Additionally, the LPN will document accordingly using the Villa forms and if needed, will be co-signed by the RN on duty. Also, the nurse may be asked to participate in any orientation for new hires and provide observations for the nursing students in the area. The nurse will demonstrate and be accountable to the Agencies Mission, Vision, Values, Guiding Principles and Strategic Plan.
ESSENTIAL FUNCTIONS :
- Actively and compassionately interacts with patients to encourage engagement in treatment after completion of detoxification.
- Administering medications, treatments, inoculations, and tests as allowed by law.
- Implements verbal physician orders and authenticates in writing, within a reasonable time, in the patient record
- Maintains a safe environment applying principles of medical asepsis and applying infection control techniques and universal precautions.
- Accurately obtains vital signs and health monitoring data.
- Executes prescribed treatments and medical interventions, including transcribing and administering routine and prescribed medications in accordance with procedures established by the Medical Director and Nursing Supervisor.
- Records patient observations and activities in patient records accurately and timely in accordance with nursing standards and organizational policy and procedure.
- Screens and provides first response, including obtaining health history.
- Performs CLIA waivered tests according to regulations
- Accurately documents nursing data in an electronic clinical record.
- Maintains confidentiality and security for all client and staff related materials and / or records.
- Demonstrates, promotes and practices cultural competency toward clients, families and staff.
- Exercises full compliance with the Justice Center’s Code of Conduct for People with Special Needs.
- Proficient in Microsoft Software such as Word, Excellus, Outlook, etc.
- Effectively communicates with a multidisciplinary team.
- Exercises full compliance with Agency Code of Conduct, all Agency policies and procedures and Federal and State laws and regulations.
- Attends regularly scheduled staff meetings.
- Follows all policies and procedures with regard to patient information protection, including alcohol-drug patient confidentiality (42 CFR) and HIPAA (45 CFR). 11.
- Attends training on chemical dependency as required by employer.
- Maintains and demonstrates a current working knowledge of applicable clinical procedures and nursing care based on company policy and State and Federal guidelines.
