Substance Use Disorder Therapist - Outpatient Clinic

JOB TITLE : Substance Use Disorder (SUD) Therapist
POSITION GRADE : 80 ($19.50-30.00 commensurate with education and experience)
FLSA STATUS : Non-Exempt
DEPARTMENT / PROGRAM : SUD / Integrated Outpatient Services Clinics
SUPERVISOR : Clinical Supervisor
JOB SUMMARY :
Under the general direction of the Clinical Supervisor, the SUD Therapist provides evaluation and treatment for individuals diagnosed with substance use disorders. The SUD Therapist will also be responsible for the maintenance of client records in compliance with oversight agencies and agency policies. This individual will carry out all job responsibilities with fidelity and accountability to the Agencies Mission, Vision, Values, Guiding Principles and Strategic Plan and commitments to the Sanctuary Model of Care.
ESSENTIAL FUNCTIONS :
- Provides individual, group, and family therapy sessions using individualized evidence based therapeutic interventions
- Completes full comprehensive assessments and screening tools with clients
- Provides family counseling and educational sessions to parents, siblings and significant others following the addiction as a family disease model to decrease the potential for relapse and the prevention of the disease for other family members
- Responsible for the maintenance of client records in compliance with Office of the Medicaid Inspector General (OMIG), the Office of the Inspector General (OIG), Office of Addiction Services and Support (OASAS), and Office of Mental Health (OMH) regulations to include, but not limited to; comprehensive evaluation, initial treatment plan, treatment plan reviews and updates, daily treatment progress notes that include individual, group and family sessions and all admission and discharge paperwork
- Utilizes evidence based SUD therapy interventions, including harm reduction model when treating clients.
- Develops an individualized treatment / recovery plan in collaboration with the client; and or parent / guarding based on needs identified in assessment and review in session notes
- Coordinates transition / discharge planning, and document specific referrals and recommendations that assures ongoing treatment needs are met after discharge.
- Makes treatment recommendations and referrals based on information and data gathered during individual, family and group sessions.
- Collaborates regularly with residential treatment providers, school staff, referral sources, community providers, and collateral contacts
- Engages in person-centered needs assessments to support linkages and referrals to other ongoing SUD services (such as MAT Medication Assisted Treatment, Peer Recovery Support, Impaired Drivers Assessment and Treatment) and community resources (including but not limited to health homes care management and housing resources).
- Travels as needed in the community to promote collaborative relationships and client centered care.
- Secures and links client and family to community resources whenever need is identified including community support groups.
- Seeks clinical supervision to develop competencies and skills in areas of professional development.
- Informs supervisor of significant events / issues related to clients as appropriate and follows program’s defined incident reporting process as required
- Participates in regular individual supervision, group supervision, program departmental meetings, case review and agency meetings
- Maintains and develops clinical and professional knowledge by completing recommended education, trainings, workshops.
- Exercises full compliance with the Agency’s Code of Conduct, all Agency Policies and Procedures.
- Demonstrates, promotes and practices Diversity, Equity, Inclusion and Belonging in relation to clients and staff through respect and understanding achieved through training, individual supervision and Agency cultural activities.
