Village Manor of Cascadia is looking for a Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals to be a FORCE For Good in Wood Village, Oregon.

Village Manor of Cascadia is a long-term care nursing facility that cares specifically for individuals with behavioral issues, as well as their physical and medical needs. We are conveniently located off I-84 exit 16, in Wood Village. We offer great benefits, as well as very competitive pay.

The LPN will provide quality care to residents in an environment which promotes resident rights, dignity, freedom of choice, and their individuality while providing individualized attention and encouraging residents to maintain or attain the highest level of wellbeing.

Position Summary :

Under the direction of an RN, the Licensed Practical Nurse (LPN) (Certified) provides person-centered nursing care and treatment to elderly, acutely ill, or injured residents. Integrates technical skills and theoretical knowledge to meet the individual needs of residents and families. Assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. The LPN (Certified) uses the knowledge and expertise gained through the special certification process to treat the resident / patient population that would benefit from the specialized clinical care.

Essential Functions

  • Provides direct care and / or assists with care as directed and consistent with their scope of practice and competency. Initiate the SBAR process and collaborate with the RN for assessment and recommendations. Makes rounds with physicians and other team members as needed. Participates in resident evaluation and care planning activities, reviews and revises residents' assessments and care plans as needed to the extent permitted by state nurse practice act. Coordinates residents' care activities and communicates changes to other team members and resident family / significant other. Oversees Medication Aide / Tech in medication / treatment administration, where applicable. Administers treatments.
  • Reviews, transcribes, communicates and implements physician orders, obtaining RN signature as required by state regulations or company policy. Communicates with resident's physician. Assists with emergencies, administers cardiopulmonary resuscitation.
  • Documents care performed and observations of resident status in the clinical record promptly by utilizing standard clinical documentation guidelines as required by the company, and local, state and federal rules and regulations. Monitors completeness and accuracy of own clinical medical record entries and those of assigned LPN and Nurse Aides daily.
  • Participates in medication management to promote optimal safety and effectiveness for residents by :

Administering medications as ordered, including next scheduled dose following admission / readmission.

  • Notifying Supervisor if medications are not available;
  • Preparing, administering and documenting medications as prescribed;
  • Observing and reporting resident responses to medication;
  • Identifying and promptly communicating adverse drug reactions;
  • Maintaining narcotic records accurately within scope of practice;
  • Completing medication interchange and insurance authorizations as assigned;
  • Ordering or arranging for ordering of pharmaceuticals;
  • Notifying physicians of automatic stop orders;
  • Completing recapitulated physician orders review as assigned; and / or,
  • Cleaning, organizing and replenishing supplies on medication / treatment carts after each use and prior to shift change.
  • Completes clinical admission paperwork for new admissions.
  • Initiates and assists consultants while consultants are providing services. Reviews documentation from consultant visit and follows up on recommendation(s).
  • Assists with follow up on results of qualitative and quantitative medical record audits.
  • Communicates and implements pharmacy review / drug regime review recommendation as assigned.
  • Assists with clinical discharge process.
  • Performs other clinical care services within scope of practice for additional certifications.
  • Ensures punctuality and regular attendance for assigned shifts.
  • Other Functions

  • Performs other duties as assigned.
  • Maintains resident / patient confidentiality.
  • Conducts job responsibilities in accordance with the standards set out in the Company's Code of Conduct, its policies and procedures, applicable federal and state laws, and applicable professional standards.
  • Requirements

    Education :

  • Graduate of accredited LPN School of Nursing.
  • Licenses / Certification :

  • Current valid LPN license in the state employed.
  • Valid CPR certification
  • Current certification in one of these clinical specific areas :
  • Long-term care;

  • Gerontology
  • Wound care;
  • IV therapy;
  • Ventilator, Pulmonary & Tracheostomy ("VPT"); or
  • Hospice & Palliative care
  • Experience :

  • Six months experience in a long-term care environment preferred.
  • All employees must adhere to COVID-19 guidelines and safety precautions, and must be vaccinated or able to provide an acceptable medical or religious exemption.

    All employees of Village Manor of Cascadia are required to submit and be cleared to work in the facility per Oregon's specific background check requirements prior to contact with patients / residents.
