CASE MANAGER AND OUTREACH SPECIALIST
The Village of McFarland has an immediate opening for the Case Manager and Outreach Specialist position within the Senior Outreach Services Department. The Case Manager and Outreach Specialist will be responsible for providing comprehensive and complex case management services to adults ages 60 and over. The objective of these services is to assist clients in solving medical, financial, and accessibility problems, thereby helping older adults to remain independent in their own homes. We serve McFarland and Cambridge as well as neighboring townships. The work involves:
- Identifying older individuals that may need access to services.
- Making personal contact to educate and assist eligible adults to make use of available community facilities, services, and resources.
- Creating and maintaining a comprehensive client file.
- Conducting functional assessments.
- Creating case plans.
- Providing service coordination.
Additionally, the Case Manager and Outreach Specialist provides opportunities for community engagement through events, presentations, volunteer recruitment and media postings.
MINIMUM REQUIREMENTS:
The qualified candidate will possess the following education and experience:
- Bachelor’s degree in human service field.
- Three (3) years’ experience working with older adults.
- Previous case management or social work experience.
- Any combination of education and experience which in the sole discretion of the Village would demonstrate the ability to meet the required knowledge, skills and abilities for the position may also be considered.
HOURS:
Monday through Friday, 8 a.m. to 4:30 p.m., with the ability to attend meetings or events in the evening or on weekends when needed.
SALARY:
This is an non-exempt, hourly position with an annual pay range of $55,098 to $69,821 (depending on qualifications and experience).
