What are the responsibilities and job description for the Activities Assistants /Skilled Nursing Facility position at Village Square Healthcare Center?
SUMMARY
Assists the Activities Director in providing an ongoing program for activities; designed to meet the interests and physical, mental, and psychosocial well being of each patient.
ESSENTIAL RESPONSIBILITIES include the following. Other duties may be assigned.
Assists the Activity Director, volunteers, and paid activity entertainers in fulfilling the needs of residents.
Conducts activity programs as planned to meet the functional levels, needs and interests of each patient as assigned by the Activity Director.
Completes appropriate records indicating patient’s attendance and participation in individual and group activities in an accurate and timely manner.
Completes assessments, MDS, Care plans and gathers information to design activities that are multi-faceted, meet patient’s functional levels, and reflect needs and interests of each patient as assigned by supervisor
Assists in the development of the Activities Calendar.
Completes required forms and documents in accordance with company policy and state and federal regulations.
Supports residents’ needs and desires through 1 : 1 interaction, small group activity and large group meetings.
Assists with off-site activity trips, when available and appropriate.
Assists residents and guests to activities with the help of direct care staff.
Helps fosters a sense of community within the facility.
Conducts job responsibilities and completes from and documents in accordance with company policy and state and / or federal regulations.
Other assigned duties and responsibilities.
QUALIFICATIONS
High school diploma or equivalent, or one (1) to three (3) months related experience and / or training.
Previous experience in a health care setting preferred.
Meets all state-specific requirements
Proficient in Microsoft Office and using a computer.
SUPERVISORY RESPONSIBILITIES
In the Activity Director’s absence, may supervise volunteers and others for whom the department is administratively and professionally responsible.
PHYSICAL DEMANDS AND ENVIRONMENT
Employee is frequently required to stand, walk, use hands and / or fingers, reach with hands and arms, feel, talk, and hear. Employee will occasionally assist to lift and / or transfer patients weighing between 100-250 pounds. Employee is occasionally exposed to blood or other body fluids, and fumes or airborne particles. WAGE RATE FROM$18.00 TO $20.00 AN HOUR DEPENDING ON EXPERIENCE.
