VillageCare is looking for a self-motivated and passionate RN with previous Care Management experience for our Care Management Supervisor position. This role is responsible for oversight of staff, productivity, the care transition program, and ensuring care plans address the health and psychosocial needs of members.

Some Daily Job Functions Will Include

• Supervise and manage the activities of care management staff, ensuring coordination and communication across members of the Interdisciplinary Care Team.
• Conduct telephonic assessments of Member's status, review and evaluate re-assessments, risk for recurring hospitalization, and member progress towards goals.
• Ensure that care plans, care management activities, and complaints and appeals are documented.
• Process requests for authorization of covered services within regulatory guidelines.
• Evaluate discharge plan and appropriateness of discharge status, in coordination with UM department.
• Develop and follow workflows for Chronic Care Improvement program and Quality Improvement program.
  
  

We would like to speak to those who bring a Bachelor's degree along with an unrestricted NYS RN license. Seeking those who have 1 years practical supervisory experience in care management within a managed care setting.

Must reside within the New York Tri-State Area - NY, NJ, or CT.
