Responsibilities
Your key responsibilities will include providing routine check-ups, conducting thorough examinations, and developing treatment plans. You will also be responsible for maintaining accurate patient records and communicating effectively with patients and their families. As a member of our medical team, you will work collaboratively with other healthcare professionals to ensure seamless patient care.
Qualifications
To be successful in this role, you must have a graduate degree from an accredited advanced practice program and be certified as a Nurse Practitioner or Physician Assistant. You should also have at least two years of primary care or home care clinical experience as an Advanced Practice Provider. Additionally, you must have a current, unrestricted State license or be eligible for licensure and a valid drivers license. You will also need to possess active Drug Enforcement Agency (DEA) registration or be eligible for registration, as well as active Medicare/Medicaid Enrollment or be eligible to enroll. Familiarity with Electronic Medical Record (EMR) systems and Microsoft Office is also required.
